Behavior Consultation
This service shall provide expertise, training, and technical assistance in any of the following specialty areas to assist family members, caregivers, and other service providers in supporting the individual enrolled in the waiver.

Personal Assistance
These services mean direct support with ADLs, IADLs, access to the community, monitoring of self-administration of medication or other medical needs, and the monitoring of health status and physical condition or work or post-secondary school-related personal assistance.

Respite Care
These services are temporary, substitute care that is normally provided by the family or other unpaid, primary caregivers who reside in the same home as the individual. These services are provided on a short-term basis due to the emergency absence of or need for routine or periodic relief of the caregiver. *These services must be requested in advance, when possible*

Companion Care
This service provides nonmedical care, socialization, or general support to adults (18 years of age or older). These services are provided either in the person’s home or in various locations in the community.

Sponsor Residential
These services mean residential services that consist of skill-building, routine supports, general supports, and safety supports that are provided in the homes of families or persons (sponsors) providing supports under the supervision of CDS, Inc., that enable an individual to acquire, retain, or improve the self-help, socialization, and adaptive skills necessary to reside successfully in home and community settings. *If interested in becoming a sponsor with CDS, Inc., please contact the office.*

Supportive In-Home
These are residential services that take place in the individual’s home, family home, or community settings that typically supplement the primary care provided by the individual, family, or other unpaid caregiver and are designed to ensure the health, safety, and welfare of the individual. These services include support with skill-building activities, routine supports, socialization, and safety supports that enable an individual to acquire, retain, or improve self-help skills.
